As a parent or guardian, I certify that (ATHLETE'S FIRST AND LAST NAME) has my consent and permission to participate in the regular practices and/or training for San Diego Pole Vault inc., USATF events, and other activities associated with San Diego Pole Vault inc. I/we do hereby assume all risks and hazards incidental to such participation. Furthermore, the undersigned parent/guardian acknowledges that the athlete is physically capable to participate, and is aware of and understands the inherent risks involved in such activity.
The undersigned parent and/or lawful guardian of the above named athlete, hereby releases and holds harmless the San Diego Pole Vault inc., head coaches, assistant coaches, team parents, sponsors, employees, Cathedral Catholic High School and all other parties of interest from all claims and causes of action, included but not limited to, all damages of any kind which may arise from or out of the above named minor’s participation inSan Diego Pole Vault inc. This also includes any amounts incurred by the club for defending against any such claim or judgment, including all attorney’s fees and costs incurred. In addition, the undersigned parent/guardian acknowledges that he/she and such child will not hold San Diego Pole Vault inc. or Cathedral Catholic High School, liable for any expenses, property damages, personal injuries and/or death sustained by such child while participating in the San Diego Pole Vault inc.
Photography and Media Release: The undersigned parent/guardian grants San Diego Pole Vault inc., its coaches, staff, and authorized representatives permission to photograph and/or video record the above named athlete during practices, competitions, and other club-related activities. I consent to the use of such photographs, videos, and digital images for promotional purposes, including but not limited to social media, website content, marketing materials, publications, and other club communications. I understand that no compensation will be provided for the use of these images and that all rights to such photographs and videos belong to San Diego Pole Vault inc. If I wish to opt out of this media release, I will notify the club in writing.
The signature below indicates that I have carefully read and understand this release and all of its contents and legal effect. I agree that this release is contractually binding, and I sign it of my own free will.
